Community Support Specialist at COMTREA, Festus, MO

COMTREA currently has a full time position open in Festus MO for a Community Support Specialist.
 

Job Summary

Provide intensive, in home case management service to reflect client level of care clinically indicated; advocacy; education re:  medication and illness related to mental health, substance use/abuse and health and wellness/assistance with medical appts and transportation arrangements; intervention strategies to keep them out of inpatient setting and in least restrictive setting.  Work with clients to attain their highest level of functioning and life satisfaction through strengths’ based intervention by means of utilizing client assets/needs/preferences. 

 

Essential Job Functions

1.  Client Services

1.1 Client contact at minimum of 25-26 weekly hours.

1.2 Pre-approved leave time by supervisor

1.3 Multi-disciplinary treatment team involvement

1.4 Education regarding medications and mental illness, substance use and abuse and health and wellness in keeping with achievement of client life satisfaction; advocacy; community integration and interaction; service coordination.

1.5  Awareness of and appropriate referrals to community based resources and other service providers (i.e. AVS, Voc Rehab,  Vocational and pre vocational supports, Public Administrator, medical professionals, housing supports, educational supports, food pantries, JFCAC, etc.)

 

2.  Chart Reviews and Documentation

2.1 Documentation as required by standards, which include, but are not limited to:  CIP, ITP, progress notes, quarterly reviews, AIMS scales, Quarterly chart reviews, and Monthly peer reviews. (For specific documentation timeframes, see CSS Job Duties and Documentation Guidelines protocol).

 

3.  Treatment Team Component

3.1 Monthly department staff meetings

3.2 Individual supervision at least every other week

3.3 Team meetings held at least twice per week

3.4 Integration with multi-disciplinary treatment team members (COD, IDDT, PSR, SCLP, Therapist, Guardian, etc.)

3.5 Assisting with client coverage for other members of the treatment team.

  

4.  Self development

4.1 Monthly field supervision with Supervisor

4.2 Complete required e-learning trainings

4.3 Educational and professional development (graduate school, trainings within and outside agency)

4.4 Any other specialization skills determined by CSS and Supervisor.

 

5.  Ethics and Corporate Compliance

5.1 Complies with all aspects of Comtrea’s Code of Conduct

5.2 Places the health and safety of clients and staff as a priority

 

Other Required Job Functions:

 1.  Community Support

1.1 Educates public about Comtrea and various departmental activities/services

 

2.  Special Projects

2.1 Optional participation in departmental activities and fund-raising events

2.2 Additional job responsibilities assigned by supervisor

 

Qualifications (Minimum required to do the job)

Education:  BA/BS/BSW (preferred) or equivalent experience

 

Experience:  1-2 years work with inpatient or outpatient seriously and persistently mentally ill clients

 

Skills:  Analysis, problem solving, documentation, keyboarding, word processing ability, basic computer skills for data input, good clinical knowledge, independence and willingness to work as a team member, ability to seek/receive supervision on a regular basis.

 

Physical and Mental Requirements:  Excellent communication and verbal skills, confidence, patience, and creativity.  Maintain current driver’s license and insurance coverage when using own vehicle.

 

Supervisory Responsibility:  None
